Cancellations

Please call the office at least 24 hours in advance to cancel appointments.

Referrals

Please call the office at least three (3) days before the appointment and provide the following information:

  • Your name
  • Doctor you are being referred to
  • Phone and fax numbers of doctor being referred to
  • Date of the appointment
  • Diagnosis for the referral
  • Your insurance
  • Is this visit to see a doctor or to undergo a specific procedure (such as surgery)?
  • If this is a surgery referral, where will the surgery be performed (office or hospital)?

Please note, we do not mail referrals to specialists. Check your insurance plan to determine whether you need to bring a referral with you. If you are unclear about the requirements for your referral, please ask a CarePlus staff member and we will help you determine what you need.

Prescriptions

Please call the office at least 48 hours in advance for all medications. For clinics that are closed on weekends, calls received after 12:30 p.m. on Friday will be returned on the following Monday. When you call, please be prepared to provide the following information:

  • Patient’s full name
  • Date of birth
  • Name of the medicine(s)
  • Prescription duration (one month, three months, etc.)
  • Your phone number
  • Pharmacy name and address

Emergencies

If you are experiencing chest pain, severe shortness of breath, severe headache or bleeding, call 911 or go directly to the nearest hospital emergency room. You can always call the CarePlus office with any questions about acute medical problems after hours. If your doctor is not in the office, you can leave a message for the doctor on call – If you do not receive a call back within 30 minutes, please call again.

Records Releases

Please complete a Medical Records Release Form to obtain your medical records from your previous family doctor and any specialists you may be consulting.
